CREDIT BUREAU DATA CONSENT

Pursuant to the Credit Reporting Agencies (CRA) Act 2010 and Central Bank of Malaysia Act 2009, by proceeding with this application within GX BANK BERHAD (“GXB”) mobile application (or any other platform GXB may provide), I/we hereby give my/our consent and permission:

1. to share in any way, with GXB, all of my/our personal data (including but not limited to sensitive personal info, Central Credit Reference Information System (CCRIS) data, trade reference info, litigation info, company registration info, credit info on me and the business/company that I represent, transaction data on the business/company that I represent (if available), any information obtained from credit bureau/agencies, and any reports related thereto) (hereinafter referred to as “Personal Data”) obtained now or that is subsequently obtained from time to time;

2. to GXB, to conduct credit checks on me/us, and extract my/our Personal Data and credit bureau report from credit bureau agencies with regards to my/our application,

for the purpose of GXB to carry out the activities below:

1. to process and complete my/our application for financing and/or other related services offered by GXB;

2. to evaluate the credit risks of myself/us for any credit related products by GXB.

I/we confirm that all information that I/we have provided to be used to extract the Personal Data are all true, up-to-date and accurate. Should there be any changes to any of such information, I/we shall notify GXB immediately.

I/we further represent that I/we have obtained consent and am/are authorised by my organization, our directors, shareholders, officers, authorised signatories, owners, partners, guarantors, individuals, and/or party providing securities, to give this consent to GXB such that consent has been obtained individually to obtain from credit bureau agencies for the purposes as mentioned herein and any other relevant information whichever is applicable
